What is physical therapy?

Physical therapy is a treatment that includes a combination of exercises, stretches, manual therapy, and neuromuscular education to decrease pain and increase mobility and function.

What types of physical therapy does Core Rehab provide:

We treat most conditions including but not limited to the following:

* Work Injury

* Auto Accident Injury

* Back Pain

* Neck Pain

* Sciatic Pain

* Joint Pain (hip, knee, shoulder, ankle, elbow)

* Post-surgical

* Sports injuries

Do I need an order for physical therapy from my doctor before I start therapy?

No. Illinois is a direct-access state, meaning patients do not need to go to their doctor before starting physical therapy. However, depending on your insurance, there may be a limit (usually 30 days) on how long you can receive physical therapy before you will need a referral from your doctor in order to continue treatment.

Is there a medical doctor there that I can see for my pain/condition?

Yes, Dr. Krishna Chunduri MD is board-certified in Pain Management and is able to see new patients as well.

What insurance does Core Rehab take?

We are in network with most plans within the following insurance companies:

- Aetna PPO
- Blue Cross Blue Shield PPO
- Cigna PPO
- Medicare
- United Health (PPO)

What should I bring to my first appointment?

For your first appointment, be sure to bring your insurance card, a photo identification and if you have a referral or doctor's order for PT, please bring that as well. If you do not have one, that is no problem. If you have any recent imaging reports such as x-rays, CTs or MRIs for the condition you are seeking treatment for, please bring those as well.

How long are the physical therapy sessions?

It depends on the injury or condition. Your first session ( initial evaluation) will usually be shorter than your treatment sessions. Most treatment sessions are close to an hour in length.

How long will I need to do physical therapy?

Again, it depends on the injury or condition and will be determined by our physical therapist. Some patients require 12 sessions or less while other conditions can require months of therapy.

How often should I be attending physical therapy?

That is determined by your physical therapist, but usually 2-3 times per week.

Where is your facility located?

Our clinic is located at 6524 W. Archer Ave. We are west of Midway airport between Harlem Ave. and Austin Ave. on the northeast corner of Archer Ave. and Neenah Ave.

Is there parking at or near your facility?

Yes, there is free parking located on the west side of the building on Neenah St. and metered parking in front of the building on Archer Ave.
